• Emergency contacts
• NDIS plan and funding information
• Information about your disability, health or circumstances that is relevant to your supports
• Notes, reports and records created while working with you

We use this information to:

• Provide the services you have asked us to deliver
• Work with your chosen providers and the NDIA when you have given consent
• Meet our legal and professional obligations
• Keep accurate records of the support we provide

Your information is stored securely and only accessed by authorised CabemCare staff and approved subcontractors who need it to do their job.

You have the right to:

• Access your information
• Ask for corrections if something is wrong
• Withdraw consent for information sharing, unless we are legally required to disclose it
• Ask questions about how your information is managed

We handle personal information in line with the Privacy Act 1988 (Cth), the Australian Privacy Principles and the NDIS Practice Standards.
